All patients should complete the Registration, Ethnicity and practice patient questionnaire forms below.
* PLEASE NOTE - WE ARE UNABLE TO PROCESS YOUR REGISTRATION WITHOUT THE COMPLETION OF ALL 3 FORMS*
**WE ARE ONLY ABLE TO ACCEPT PATIENTS WITHIN CERTAIN OLD TOWN AREAS - PLEASE ASK AT RECEPTION FOR MORE INFORMATION ON THE PRACTICE CATCHMENT AREA**

In addition, for patients aged 16 and over complete the following form:

Please return completed forms to the surgery. Each patient should have 3 completed forms and ensure you have signed the Registration (GMS1) form.
Notification of Change in Personal Details
Requests for the change of a patients name will only be processed when one of the following documents are shown at the surgery:
- Marriage Certificate.
- Change of name deed poll document.
Patients who have never registered with a GP in the UK before will need to bring a copy of their passport & include on the registration form the date that they came to live in the UK.
Registrations will not be approved without the necessary documents being produced.
